SUBMITTING A REQUEST FOR INVESTIGATION

  1. Download the Code Compliance Request for Investigation Form. (The form will download, and should open in Adobe Acrobat Reader.)
  2. You can fill out the form right on your screen and then print it. Or print the blank form and complete it by hand.
  3. Be sure to SIGN the complaint request form as unsigned complaint requests will not be processed.
  4. Fax it to 530-265-9851 or deliver/mail it to:

CDA is open to the public Monday through Friday (excluding County Holidays), from 8:00 AM to 5:00 PM including the lunch hour.

Permits are not accepted for processing after 4:00 PM.

CDA accepts cash, checks, cashier's checks and credit cards.
